The Network Office now maintains a log of any spinal issues. This is part of the governance process embedded within the Operational Delivery Networks. The term "issues log" is used to describe spinal related risks, issues, incidents, preventable deaths and its purpose is to ensure accurate, timely investigation and resolution about issues reported to the Operational Delivery Network office from any organisation within the region.
