Changes to the Trauma Related Issues Database (TRID) 

As part of the national changes to NHS England's functions and the transition of responsibilities to the Department of Health and Social Care (DHSC), Integrated Care Boards (ICBs) and provider organisations, Operational Delivery Networks have been asked to review their functions to ensure resources are focused on activities that add the greatest value, support regional collaboration and deliver agreed national and local priorities.  As part of this review, the West Midlands Critical Care Network has considered the purpose and effectiveness of the Critical Care & Trauma Related Issues Database (CCRID/ TRID) processes, the West Midlands Adult Critical Care & Trauma Network has taken the decision to discontinue the CCRID/ TRID reporting process with immediate effect.

Whilst originally developed to support operational issues across organisations during the establishment of regional pathways, the process has evolved beyond its intended purpose into undertaking complex governance investigations. These responsibilities are more appropriately managed through established local governance arrangements, allowing the Network to focus on regional oversight, system-wide risks and shared learning.

Going forward, all patient safety incidents and concerns should be managed through the relevant organisation's local governance processes (e.g. Datix or equivalent reporting system). Where an incident involves more than one organisation, it is expected that the respective governance teams will work collaboratively to undertake any cross-organisational review, share information appropriately and agree learning through their existing governance arrangements.

The Network will continue to gain assurance through its existing governance arrangements, including governance meetings, subgroup meetings, audit, peer review and routine engagement with organisations. These mechanisms enable the identification of regional themes, pathway risks and opportunities for shared learning, while responsibility for incident investigation and organisational learning remains with provider organisations.
